Applying for IBC Review and Approval
Use these application forms to apply for IBC approval to conduct research or teaching activities with rDNA, infectious agents, and/or biologically derived toxins.
The following items must accompany the completed form:
- Biological Decontamination and Spill Clean-up Plan Template
- Biological Waste Disposal Plan Template
- Other Required Attachments Requested in the Application Form
HRPP documents are protected since checkboxes and dropdown menus are utilized. If you are trying to use certain Microsoft Word features such as spell check, the document must be unprotected.

IBC Annual Continuing Review Form
- Complete this form if you received an email titled, “IBC Continuing Review Notice.”
IBC Three Year Renewal Form
- Complete this form if you received an email titled, “IBC Three-Year Renewal Notice”
- If you plan to renew your project, a renewal application should be submitted two-months prior to the expiration date of the current project to avoid a lapse in IBC approval.
Reporting Research Related Incidents Form
Incident Report Form
- Use this form to report any research-related accidents or incidents involving Potentially Hazardous Biological Agents.
- If the accident involves recombinant DNA, it must be reported to the IBC office

Request IBC Approval for a Change in Protocol
Amendment Request Form
- Researchers have the option of using this Amendment Request From or a cover letter for submission of a change in protocol. Both options must be submitted by the Principal Investigator.
Adding/Removing/Changing Personnel on an IBC Study

- Complete and submit this form to request personnel changes on an IBC study.
